ELECTRONIC DISCLOSURE AGREEMENT

By clicking "yes" below, you agree that you have read and agree to be bound by the terms of this Electronic Disclosure Agreement.

1. ELECTRONIC DISCLOSURE NOTICE:

All documents, including but not limited to, this Electronic Disclosure Agreement, the The Media Game, LLC Legal Terms and Conditions, the The Media Game, LLC Privacy Policy, and other documents which may apply depending on whether you are a Preferred Client or an Independent Distributor, such as The Media Game, LLC’s Preferred Client or an Independent Distributor Application & Agreement, as well as all notices, disclosures and records (collectively, “Disclosures”) relating to your account with The Media Game, LLC will be sent to you electronically at the email address indicated by you in the account registration form or by viewing the Disclosures on the internet. Alternatively, The Media Game, LLC may provide you with Disclosures via fax or in the mail.

2. CONSENT TO ELECTRONIC DISCLOSURES:

By clicking “yes” below, you agree to receive all Disclosures from The Media Game, LLC electronically by email or by viewing the Disclosures on the Internet.

3. WITHDRAWAL OF CONSENT.

You may withdraw your consent to electronic Disclosures in the future at any time without charge. To withdraw your consent, please email us at info@TheMediaGame.com and indicate that you wish to receive all future communications in writing. We will send you a confirming notice once we have processed your change request.

4. TECHNICAL REQUIREMENTS:

To access Disclosures electronically and print copies of these Disclosures for your records, you must have internet access, as well as access to a printer. By clicking “yes” below, you confirm that you have access to a computer that is capable of accessing the Internet operating Internet Explorer or Netscape Navigator, version 4.0 or greater, or some other internet browser that enables you to access The Media Game, LLC’s web site (www.golfsurvivalguide.com). You also confirm that you have access to a printer or the ability to make hard copies of your information and records. Please make a copy of The Media Game, LLC’s Electronic Disclosure Notice and any other documents listed in paragraph 2 above for your records. To print, click on the underlined document, select “File” on the menu of your browser, and then select “Print.”
